OVERVIEW OF SERVICES

  • Provides support, non-medical information, and assistance to patients and families
  • Serves as liaison between the hospital and patients/families. Patients may contact the Patient Relations Coordinator if they have any concerns or if they’d like to offer positive feedback.
  • Provides Physician Hotline phone number (408) 283-7603 for physicians to call from the hospital, office or home regarding their concerns
  • Coordinates Patient Satisfaction Program for hospital
  • Provides notary services for patients and families
  • Handles lost property for patients
  • Coordinates Translation Program at hospital (Language Line and outside translators) for non-English speaking and hearing impaired patients, families and visitors
